On 20 July 2026 Baltimore's University of Maryland Medical System named a permanent general counsel after a year of interim coverage, resetting client walls across 11 hospitals that deliver 25 percent of Maryland hospital care. The American Medical Association reported in 2026 that Johns Hopkins Health System held 26 percent of Baltimore-Columbia-Towson admissions and University of Maryland Medical System held 25 percent. Maryland's AHEAD Implementation Period began on 1 January 2026 after the 12 November 2025 Amended and Restated AHEAD Model Maryland State Agreement; Medicare fee-for-service rate-setting authority moves to CMMI in 2028, while HSCRC keeps global budgets for other payers. UMMS's FY 2025 audit states that standard charges at the Medical Center and named member hospitals reflect rates regulated by the HSCRC. Construction-in-progress at UMMS was $459.114 million at 30 June 2025, with remaining contractual construction commitments of about $317.775 million.
The Johns Hopkins Hospital listed 1,146 licensed beds in the August 2023 Johns Hopkins Medicine overview. MedStar Harbor Hospital, Sinai Hospital of Baltimore, GBMC in Towson, Mercy Medical Center, and Kennedy Krieger Institute are the other named in-house employers. A chief legal officer at a multi-hospital community system said the seat needed someone who had already lived through an HSCRC rate filing. Kennedy Krieger created its first in-house general counsel seat on 11 February 2021.

In 10 closed in-house searches in Baltimore over 18 months, Sartori found 7 hires required prior HSCRC rate-setting work before a board would shortlist. Johns Hopkins Health System still lists its February 2022 general counsel; LifeBridge Health lists a senior vice president and general counsel covering Sinai, Levindale, and Grace Medical Center in Baltimore City; MedStar Health publishes an executive deputy general counsel as chief counsel for all MedStar hospitals, with hospital counsel covering MedStar Franklin Square, MedStar Good Samaritan, and MedStar Union Memorial together.
Legal headcount follows the operating split. On 22 October 2025 UMMS named an inaugural Executive Vice President, Community Health Division, over seven community member organizations, beside an Academic Health Division led from University of Maryland Medical Center's two Baltimore City campuses. UMMS closed a national general counsel search on 20 July 2026 after a year of interim coverage. The predecessor moved to a member-hospital presidency on 18 July 2025. On 22 October 2025 the system stood up a Community Health Division over seven member organizations, so the general counsel now covers two operating divisions.
